Cracks, they often have a use by date, check its totally flat, and store them flat and not with anything heavy on them etc.

Centrifugal force can expand a crack. Add some friction on the edge and it looks like the disc peels apart in slo-mo. Like a tire blowout.

Basically always use the proper disc for the job (cutting for cutting, grinding for grinding), keep rpms within specs, and don't push (1)

The tool is called an angle grinder, but the disk is a cutting disk. When you use a cutting disk to grind, it bends and then shatters.
