Well, what we can be reasonably sure about is that there is a lot of reason to think it's not 2 sexes and 2 genders and one sexuality. So, start with that, and we can work on sorting out how much more we need to account for in order to keep people healthy and happy.
The idea of "well, science isn't 100% on this, so let's just stick with 'male/female' and 'straight' as the only real things to based medicine on" gets a lotta lotta people extremely dissatisfied and/or dead.
