It was probably stuck down in a crack and you picked it up, if you ride the center where cars don't normally drive you'll pick stuff up.

Can happen anywhere. Be particularly careful changing lanes on bridges and overpasses. They tend to have slits that prop nails vertically.

My guess is that it was stuck in the road, end up, and the weight and quickness of the bike did the rest
