Roots will follow water. Make the irrigation system superficial, the roots will be superficial. Not enough water? Roots become brittle.

In this case, it's because all the roots go down instead of out. There's no strength to hold the tree from horizontal forces like big gusts.

it's most likely partially dead due to a disease in the area. my street has a problem like that which kills our trees from the inside

The canopy grows the same size as the root system, so if the tree keeps compact, it'll live. But yes, very shallow.

It's as you say. You can tell where it broke from its roots by the color of the wood there, since trees to be a lighter shade on the inside.
