Milley actually went around Trump and inserted himself into the nuclear chain of command. Even though technically he was only to advice the POTUS. There was no hemming and hawing over technicalities. He went to them and the answer was "Yes sir.". They don't actually put the dumb ones in charge of the nukes. It's implicitly understood that the order is to be refused.
