In my neck of the woods, it’s done under sedation. I’ve had several colonoscopies, and I’ve watched the procédure throughout (I think), and been very comfortable. The recovery nap is epic.
This reminds me of a couple on a Vespa, who was towing their baby in this type of bicycle carrier. It flipped over when they turned a corner. I called 911 on them.

How is the water propelled sideways though? And at that pressure.
